Event Part of National Campaign Focusing on Health Centers as Local Solutions for Communities.

Tri-City Health Center (TCHC) was awarded a $15,000 grant by Kaiser Permanente Community Benefit Program in southern Alameda County for the TCHC PHASE Plus program.

The San Francisco Foundation (TSFF) Board of Trustees awarded Tri-City Health Center (TCHC) a grant in the amount of $40,000 to expand its capacity to serve low-income, uninsured Hispanic immigrants who suffer from diabetes and hypertension.

Tri-City Health Center (TCHC) was awarded a $50,000 grant by Kaiser Permanente Community Benefit Program in southern Alameda County for the second year expansion of the "My Choice, My Future" program.

Tri-City Health Center has been awarded two grants of $10,000 each by Kaiser Permanente for the Making A Difference Everyday (MADE) and Team Up for Health programs.

Tri-City Health Center (TCHC) has been awarded a $30,000 grant by The Boston Scientific Foundation for the Homeless Outreach for People Empowerment (HOPE) project providing health care and social services to the homeless population in Alameda County.

TransVision and Tri-City Health Center will host Alameda County’s first ever Transgender Health and Resource Conference providing opportunities to network and access services and resources. The event is free and open to the transgender community.
